See a dentist. There may be other problems contributing to bad breath,
which are not addressed by home brushing. There are additional
modalities, such as flossing, and stimudents. (I was having some
incipient gum problems before my dentist got me to using stimudents -
they are even habit forming.)

What Rane said. Not all that smells bad is from teeth. Gums and tonsils
are possible offenders. Diet may also play a role (some substances are
excreted in breath, sweat, and similar not-usually-thought-of ways).

There is a product called Breath Assure which is supposed to deal with bad
breath internally, rather than on an external basis. I have never tried it,
but I have heard it works well.

to kill germs, try one of the listerene mouthwashes. it comes in
several flavors now.
it may sting your mouth though.
hubby had a gum infection, and also had a lot of plaque under the
gumline. he has some fancy stannous flouride toothpaste, and has to
use that before he goes to bed. in the morning he has to use a
bakingsoda toothpaste. and he has to use listerene too.
and floss.
anyway, those peridontal germs are contageous, according to the
dentist. so i have to do the listerene thing too.

Any drug store like Rite Aid or F&M, or perhaps Meijer. No idea of it's
contents, but you could always ask the druggist.
